Our patient portal provides free, secure access the most accurate, up-to-date information about your care at Shamrock General Hospital and Shamrock Family Care Clinic, and it connects you to your health care team.

With our patient portal, you can easily take a more active role in your care!

  • Request appointments.
  • View and download your medical history.
  • Send a summary of care document to another caregiver or provider.
  • View upcoming appointments, complete with location directions.
  • Receive and print laboratory, microbiology and pathology results 24 hours after completion.
  • Receive and print diagnostic imaging reports 24 hours after being signed by the radiologist.
  • View a record of your current medications, allergies and conditions.
  • Grant record access to others who may assist with managing your care.
  • Send messages to your provider regarding your care.
  • Request medication renewals of current prescriptions.
  • View and print education or instructions related to your care.
  • Access your medical information via your smartphone using the mHealth app.

First, let clinic staff or the hospital business office know your interest in signing up for the patient portal.  They can then assist you with the first steps to access the portal.

All of the information contained in your profile on the patient portal is information from your Shamrock General Hospital and Shamrock Family Care Clinic electronic health record.

Use of the patient portal is provided free of charge to any patients of Shamrock General Hospital and Shamrock Family Care Clinic..

In addition to having access to your own health information, the patient portal also allows you to grant others, as you wish, to have access to your health information. Individuals at least 18 years old can be given proxy access to your health records with an authorized consent to help manage your care.

Your information on the patient portal comes directly from your electronic health record. If you find errors, please ask your physician’s office to correct any inaccurate information at your next clinic visit.

Yes, you may submit certain health data elements by using the Health Tracker features of the patient portal. Once you log into the patient portal, there will be further instruction on how to link your health devices to the patient portal.

If you have technical problems with the patient portal, please contact hospital business office or clinic manager for assistance.

The patient portal can be accessed from any personal computer, tablet, or mobile device with a modern web browser such as Safari, Chrome, Microsoft Edge, or Firefox.  It is best practice to be using the latest version of your web browser to ensure compatibility.

Additionally, Android or iOS users can access the patient portal through the “mHealth” app.

Once you have installed the app, you will need to sign into your patient portal account.

The patient portal provides secure electronic access to portions of your medical record, enhancing communication between patients and healthcare providers. While this tool offers significant convenience, certain precautions are necessary to minimize risks. By creating an account, you acknowledge that you have read, understood, and agreed to these Terms of Use.
 
The patient portal is not intended for emergency use.  For emergencies, contact 911 or visit the nearest emergency facility.
 
Access and Participation
  1. Voluntary Use: Access to the patient portal is optional and is not a requirement for receiving care. Your provider will not condition treatment on your use of the portal.
  2. Eligibility: The portal is available only to established patients who have had a previous visit.
  3. Account Creation: By creating an account, you agree to provide accurate, current, and complete information.
  4. Termination of Access: Shamrock General Hospital and Shamrock Family Care Clinic may suspend or terminate your access to the patient portal at any time, including but not limited to the following reasons:
    • Your healthcare provider determines continued access is not in your best interest.
    • Your account remains inactive for at least one (1) year.
    • Abuse or negligent use of the portal is detected.
 
Features of the Patient Portal

The patient portal provides access to:

  • View and print portions of your Electronic Health Record (EHR), including visit summaries, medication lists, treatment history, and lab results.
  • Update your User Profile information.
  • View and print another adult’s EHR as their legal representative with their explicit permission.

Limitations:

      • The portal is not intended for diagnostic or treatment purposes. Diagnosis and treatment can only occur during an in-person consultation.
      • Requests for narcotic pain medications or refills for medications not currently prescribed by your physician will not be accepted through the portal.

 

Privacy and Security
  1. Confidentiality: You are responsible for maintaining the confidentiality of your username and password. If you share your login credentials, Shamrock General Hospital and Shamrock Family Care Clinic are not responsible for any resulting breaches of your medical information.
  2. Password Security: If you suspect your password has been compromised, promptly log in to the portal and update it.
  3. Privacy Practices: Shamrock General Hospital and Shamrock Family Care Clinic will safeguard your medical information according to our Notice of Privacy Practices.

 

Proper Use

You agree to:

  • Use the patient portal only as intended and described in these Terms of Use.
  • Report discrepancies or errors in your medical record immediately.
  • Refrain from attempting to harm, bypass, or misuse the portal’s security features.

  1. Service Availability: The patient portal is provided “as-is” and availability cannot be guaranteed at all times. While we will make reasonable efforts to address faults, uninterrupted service cannot be assured.
  2. Accuracy of Information: Shamrock General Hospital and Shamrock Family Care Clinic strive to keep your medical record accurate. However, we cannot guarantee completeness or accuracy and encourage you to report any discrepancies.
  3. Liability: Shamrock General Hospital and Shamrock Family Care Clinic are not liable for issues arising from:
    • Technical errors or service interruptions.
    • The accuracy of information displayed in the patient portal.
    • Breaches caused by user actions, such as sharing login credentials.
